Community Based Services

Lean on Me Family Services, LLC offers interventions designed to help parents and caregivers resolve common problems and crises that may impact their family’s ability to thrive. These services aim to improve relationships and social functioning while preserving a family’s ability to stay safely together or to promote reunification.

We work with children and families in their own homes and communities, where services are most effective and convenient. Research clearly shows the positive impact of conducting services in families’ homes as opposed to an office setting. Home is where challenges exist and where change must occur. Services are as follows:

Parent Coaching

Parent Coaching is used to correct specific deficits in parenting. This service is provided in a one-on-one setting, this service tailors it’s priorities to meet each parent’s specific needs. Our team works with the parent/caregiver and child (when appropriate) to develop a specialized treatment plan to address individual problematic behaviors that impact the home, school, and community spheres. Specific examples targeted in a treatment plan might include individualized behavior management techniques or helping the parents/caregivers understand a child’s specific mental or physical health condition. Lean On Me Family Services utilizes evidence-based curriculums.

Adult Life Skills Coaching

Adult Life Skills can be utilized in multiple ways. We work with parents and caregivers with skill building to include job seeking and maintenance, house hunting, using public transportation, cooking, cleaning, budgeting, paying bills, and technology. We can assist parents and caregivers with understanding of general housekeeping tasks to ensure a safe home environment for their family. This service focuses on demonstrating and educating families on how to meet specific parent/caregiver needs.

Mentoring

Mentoring is targeted to youth ages 7-17. We focus on creating trusting relationships that the clients can build upon. We provide the tools necessary to overcome challenges, succeed in school, enhance their community involvement and develop concrete life skills. Some of the areas we focus on are study skills, social skills, time management, creative problem solving, stress management, and career development.

Family Reunification Program

This is an all-inclusive package for families in which children are currently in foster care and parents/caregivers need intensive services for reunification to be successful. This package includes weekly random drugs screens, parent coaching, supervised visitation, adult life skills coaching, FPM appearances, transportation services, treatment team meetings, and court appearances. We take the burden of setting up and supervising visitation and random drug screens for our case managers.

Family Preservation Program

This is an all-inclusive package for families that are at immediate risk of removal. We help parents/caregivers identify and build upon their strengths and resolve problems at home. The program teaches parents to advocate for their children in school and other public settings and connects families to community resources and support networks that can help. The goal of Family Preservation Program is to increase self-sufficiency within the family and reduce their reliance on public agency intervention. This package includes weekly random drugs screens, parent coaching, supervised visitation (if needed), adult life skills coaching, FPM appearances, transportation services, treatment team meetings, and court appearances. We take the burden of setting up and supervising visitation and random drug screens for our case managers.
